Whether you’re a payor, provider or government entity, social determinants are a critical component of health care today and play a key role in addressing health equity. Social Determinants of Health (SDOH) are defined as “conditions in the environments in which people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning and quality of life outcomes and risks”. ​

There are certain steps that can be taken to improve the conditions in which individuals live. The conditions often considered social determinants, or drivers, of health range from access to healthcare services to neighborhood and physical environment. Disease prevention and health promotion must not and cannot be confined to the walls of our care providers.
